Application for Assistance

Qualifications

(PLEASE READ PRIOR TO DOWNLOADING APPLICATION)


1. Must be a Single Mother who is working and/or going to school. (*Must show proof of income or proof of attending Institute of Higher Learning.)

  • Single mothers with children under the age of 21 are eligible to apply.
  • For adult children 18 and over must provide proof that this individual is attending high school or college.
  • Clients receiving disability will be considered based upon discretionary evidence and/or special circumstances. 


2. Must have a residential lease in client’s name only. (Must accompany the application, *No exceptions) 


3.  Must have a State ID. (Driver’s License, State Issued ID Card)


4. Must have copies of Social Security Cards. (Mother & All Children)


5. Must have copies of each Child’s Birth Certificate.​

All Furniture is gentle used with the exception of mattresses!
